Set Up Care At A Health Care Home
People who get their primary care through a health care home are happier with their care, have fewer emergency room visits and fewer absences from work and school.

Many health problems can be prevented, corrected or reduced if found early and quickly diagnosed and treated. Whether you see a doctor frequently or rarely have medical problems, it is important to have a health care home. This is important for preventing chronic illness, keeping immunizations and screenings up-to-date and coordinating care with specialty medical providers. Having a health care home can improve your health outcomes and quality of life.

Family Care Health Centers wants to be your health care home. You and your family will get a full range of primary health care services at our centers. Plus, these services will be affordable and easy to access. We have two state-of-the-art facilities in the St. Louis metropolitan area to serve you.

By the end of 2011, all Family Care providers will begin using an Electronic Medical Record.
